The SPCCNT aim to work in close liaison with you and your Primary Health Care Team:

• support you and your carers at home

• inform and counsel you and your carer about issues relating to your illness

• advise on symptom control

• advise you on the desirability of hospice admission, Day Hospice attendance, referral for   complementary therapies

You should note that the hospice does not have the facilities nor resources necessary to undertake advanced cardio-pulmonary resuscitation. However, in emergencies, all other appropriate measures will be taken.

How We Work

We are employed be and based at Wigan and Leigh Hospice and work in the community throughout Wigan Metropolitan Borough. We normally work geographically and individual nurses generally carry sole responsibility for their own patients and families but may need to share this sometimes eg. over annual leave periods. We perform the same functions as Macmillan nurses do elsewhere offering, on the basis of assessed need,:

• specialist palliative care nursing advice on symptom control

• specialist psychological and spiritual support for patient and carers

• information about your illness, treatment and medication where you request this

• a bereavement service

• patient and carer advocacy where appropriate

• palliative care education for patients, families, carers and other health care professionals

Messages can be left at any time, 24 hours a day and will be responded to as soon as possible.
